General exclusions for all health plans:

1. Services related to cosmetic surgery, weight reduction surgery, obesity treatment, speech disorders, learning difficulties, behavioral and developmental problems are not covered.

2. Non-medical counseling, nutritional counseling, herbal drugs (Alternative/Unorthodox medicine), food supplements, experimental drugs, virility-enhancing drugs, and assisted reproduction services are excluded.

3. Orthodontic services and dental implants are not included in the coverage.

4. Treatment of mental, emotional, or functional nervous disorders, psychological testing, substance abuse treatment, self-inflicted injuries, and suicide attempts are excluded.

5. Artificial ventilation and the use of ventilator services are not covered.

6. Transplant surgery, joint replacement procedures, and prosthetic limbs are excluded from coverage.

7. Post-acute rehabilitation services at home, domiciliary services, and the supply of durable medical equipment/appliances are not included.

8. Management and correction of congenital abnormalities are excluded from the plans.

9. Consultations with unregistered and unrecognized clinicians are not covered.

10. Eye surgery solely for the purpose of correcting refractive defects such as myopia, astigmatism, and presbyopia is not included.

11. Any service, treatment, procedure, or investigation not explicitly listed as covered is excluded from the plans.

12. Conditions caused by an act of war, an epidemic, or the enrollee participating in a riot are not covered.

13. Professional sports-related injuries and wilful exposure to needless danger are excluded.

11. Artificial reproductive techniques and complications arising from them are not covered.

12. Management of autoimmune disorders and the use of immunotherapy/immunomodulators are excluded.

13. Expenses related to school admission tests are not covered.

14. Any service, treatment, procedure, or investigation not explicitly listed as covered is excluded from the plans.
